Silver Medical Staffing is Hiring a RN Case Manager Near Carlsbad, CA
- Hourly Rate: $45-55/hour
- Full-Time Schedule (40 hours/wk): Monday - Friday, 8:30-5pm, with one Saturday OR Sunday every 6 weeks (RN Case Manager will trade one weekday with a weekend day to maintain a 40 hours/wk schedule)
- Drive to patient homes (Patient Load: 16-18/ ~20 visits/wk / avg. 4/day) to assess and administer care to patients (mileage reimbursed for all home care visits)
- Follow the hospice care plan, provide skilled and palliative care and coordinate the services provided to your patient(s)
- Cooperate with the attending physician, facilitys Medical Director, and other members of the interdisciplinary hospice team
- Manage an average patient load of 16-18/visits/wk
Requirements
- Active, unencumbered RN license to practice in the State of California
- CPR certification
- Preferred: 1 year of RN experience
